Note: No application will be considered unless all information is provided.  Upon completion of the project, we will require a signed statement to confirm the money has been directed to what it is intended as per the application.

A) THE SIGNED AND COMPLETED AUTHORIZATION FORM
B) COPY OF LAST AUDITED FINANCIAL STATEMENTS
C) A COPY OF MOST RECENT FINANCIAL REPORT
D) A LIST OF DIRECTORS/BOARD MEMBERS AND OFFICERS
E) A COPY OF CURRENT BUDGET
